Admission Requirements


  1. For New Students/High School Graduates:
    • Form 138/137 (Original and two photocopies)
    • Certificate of Good Moral Character
    • Two (2) copies of recent 2" x 2" colored photo
    • Two (2) photocopies of NSO Birth Certificate
    • Medical Certificate
    • If married, three photocopies of NSO marriage certificate
  2. For Transferees:
    • Transfer credentials/honorable dismissal
    • Certification of grades
    • Certificate of good moral character
    • Two (2) copies of recent 2" x 2" colored photo
    • Two (2) photocopies of NSO Birth Certificate
    • Medical certificate
    • If married, three photocopies of NSO marriage certificate
  3. Second Courser
    • Official Transcript of Records
    • Certificate of Good Moral Character
    • Two (2) copies of recent 2" x 2" colored photo
    • Two (2) photocopies of NSO Birth Certificate
    • Medical certificate
    • If married, three photocopies of NSO marriage certificate
  4. Cross Enrollees:
    • Written permit to cross enroll from the school registrar where student is currently enrolled
  5. Foreign Students:
    • Duly notarized letter request
    • General Application Form (BI Form No. MCL-07-01)
    • Original Copy of Medical Certificate issued by the Bureau of Quarantine and International Health Surveillance or a government medical institution with competence to certify that the applicant is not afflicted with any dangerous, contagious, or loathsome disease and is mentally fit
    • Photocopy of applicants passport showing its bio-page
    • National Intelligence Coordinating Agency Clearance
    • Bureau of Immigration (BI) Clearance Certificate
    • Alien Certificate of Registration (For Renewal of Student Visa Only)
    • School credentials
    • N.B. All documents should have Red Ribbon and stamped by Philippine Embassy in Country of Origin. Please refer to the Guidebook for Foreign Students, prepared by the Commission on Higher Education (CHED). The guidebook describes the rules and procedures governing the admission, entry and stay in the Philippines of foreign students.
